Once you're sure that your site is ready for adsense you can follow this link

Fill your details and click on the Button "Click Here To Apply". After applying you have to wait for the approval email from adsense team. You will normally receive this between one day and one week hours after submitting your application.
If your site get accepted you will get a mail like:
Congratulations! Your Google AdSense application has been approved. You'll soon begin to see relevant Google ads appear on -------------------. |
If your site is not accepted you will get a mail like:
